NEW PATIENT INFORMATION

During your first visit our goal is to get to know you better and to become more familiar with your past dental experiences.  Initially, we will gather data about your medical and dental history including information about your overall physical condition, allergies, prescription drug usage and your personal dental wants and needs.

Next, one of our doctors will perform a thorough examination recording past dental treatment and the current condition of all teeth and soft tissues.  Each exam includes low radiation panographic x-rays as well as bitewings.  We will then discuss our findings and your treatment options, so that you may participate in developing a customized treatment plan that is right for you.

Before your first visit, you may download and complete our Patient Medical Information Form (133Kb PDF).
